MRI Perinatal infarct at birth

MRI report: “encephalomalacia/gliosis involving the left frontal-parietal lobe/operculum and insula with  apparent ASI hypoperfusion. a smaller left superior teuporal gyrus and mild wallerian degeneration. slight asymmetric rotation of the left hippocampus.”


Basically, this patient had an undiagnosed stroke while in the womb resulting in seizures, the imaging essentially shows how the brain is connected by “cob webs” around the abnormal left side gap.



EEG related history:

  • Sz Onset: 12 years old

  • Aura: right hand tingling and numbness, loss of control

  • Semiology: impaired awareness, nausea/ dizziness

  • GTC x 1

  • Post-ictal: fatigued

  • Triggers: stress, sleep deprivation

  • Duration: seconds to 25 minutes

         > Frequency: 1-3x/week

  • Can Cluster: Last time was a few years ago


This is a phase II sEEG candidate:

this is an example of a sEEG seizure from this patient. No other clinical signs. Duration approx 2min with post ictal confusion
